If I have a file named “Filename.doc” what would cause me to get a copy of the same file in the same folder named “Copyfilename.doc”? This has happened to me many times and I know it is something I am doing accidentally while trying to copy a file to another folder but don’t know what it is I am doing to cause this to happen.

Are you sure it's Copyfilename.doc and not Copy of filename.doc?
The latter could be created by accident
- If you drag Filename.doc with the Ctrl key held down, and release the mouse button while still in the same folder, or
- If you select Filename.doc and press Ctrl+C, then press Ctrl+V while still in the same folder.
