It doesn't do that for me.

That might be because I don't have an avatar, but I'm not sure.

I looked for the avatar before I realised the reason why.

What would occur If I changed to get Notifications only instead of by Email?

The small avatar has nothing to do with that. If you receive a non-email notification, a number will appear to the right of Notifications, and if it concerns a private message, to the right of Private Messages. The number signifies the number of unread notifications/private messages.
